You choose how many humans (Sims, in their universe) you want in your family (from 1-8 I believe), and customize the appearance of each and every one of them. The appearance isn't the only thing you customize however. You can also decide on the personality of each Sim, along with their preferences and such.
After creating your Sim, you must buy a house for you Sims or create your own. Once you've done that, you can furnish it however you like. You've got the keep an eye on your currency, though.
Then, you've got to guide them. They each have attributes which you should try to keep as high as possible as often as possible. Some include Hunger, Energy, Social, Fun, Bladder, etc. The higher the state of their fun for example, the happier they'll be.
