Which one would be the best one to start? Something in the middle of the (touhou) difficulty curve...

I'd personally recommend that you start with Ten Desires; it's one of the easier entries in the series.

Ten Desires is pretty much the easiest game in the series but starting off with the easiest one isn't really going to help you with the difficulty of the others.
It's widely accepted that a good starting point is Embodiment of Scarlet Devil, the 6th game in the series, and the 1st one for Windows.

(But the sixth one doesn't have a visible hitbox like the rest of the Windows entries)

Got to stage 3 boss and died (Hard). The hitbox was a little weird, but more often than not, that saved me.

That tiny hitbox is standard in danmaku games; you wouldn't be able to perform rain-dodging feats otherwise

I'm aware that this is way late, but either Perfect Cherry Blossom or Imperishable Night (7 and 8 respectively) might be suitable. Both are still reasonably difficult, but have player-friendly mechanics to make things easier to learn.
On a side note, has anyone tried the Len'en series of games?
They're games made by Jynx (he made a few remade PC98 teaser things) that are the same sort of bullet hell, but with new characters and such. It can be inconsistent difficulty-wise at times (and its tougher overall), but they're really fun. The first game is, anyways - haven't tried the second yet. Both are free to play (Evanescent Existence and Earthen Miraculous Sword).
Absurdly Extra from the first game, or a beyond-lunatic version of Extra. Absurd indeed - hop to 3:55 for the boss itself.
