1) As a child, I used to play, and love, the 2nd worst game in history, Bubsy 3D for the PS1. It was something I really enjoyed playing (I had no idea of the concept of what made a game good or bad). Unfortunately, something royally hindered my experience. In Level 2 (and in one of the later levels), there is an area at the back in relation to the start, in which the level's goal is high up on some floating platforms, with a telescope thingy on the ground about (maybe) a hundred feet away. What is in-between???
Look at this video [IF YOU DARE], and see what shows up at 7:01.

There. Congratulations on your vomit. [/ramble]
2) The eel of Super Mario 64, when I was a kid... obviously.
3) This one is unusual: In Bomberman 64: TSA, in Warship Noah, that thing that kills you instantly (unless you hurt yourself) in the room before the fight against the "possessed space pirate guy."
I have no idea why this ♥♥♥♥ scared me. It is just a thing floating above your head.
What's creative about that thing is that it, graphically, fully resembles the enemy's purpose. But, how does the enemy do it in the first place? Dark magic? :3
4) In Bomberman 64, when you are vaporized by the laser. (as a kid)
5) In the same game, but when you are flattened by the mine carts in the 3rd Lava stage. :3
... (as a kid)
6) Also when I was a kid: In Banjo-Kazooie, in Rusty Bucket Bay, the shark that appears at one of the level's corners. I was, afterwards, then afraid of the one found in Treasure Trove Cove.
7) Banjo-Tooie: the huge angler fish thing. (dik a sa)
8) In Q-Bert, you might be able to guess what scared me.
