well, the first game she appeared in was Super Mario Bros. 2 for the NES. she served as the boss at the end of every level, getting harder after every defeat.

as to her relationship with yoshi is none. she is not related to him and as far as i know they do not even know eachother.

Actually, if you want to be a shtickler about it, Birdo first appeared in Doki Doki Panic, the game off of which Super Mario Bros. 2 (NA) is based.
