I thnk Fourth Level Designer Contest should involve a sky themed level. Personally.

I second NYManiac's Pipe Maze suggestion (actually, just "pipes"; the maze part should be optional ; ) ). Unlike themes such as "desert" or "volcano", there is no specific tileset or background that you must or should use to make it look right. This leaves a lot of room for creativity.

A city level or a fortress level like with the city level a mixture of things could be going on such as war or even there being a giant battleship above the city and the fortress could be in the sky or even be underground and such in other words one idea can branch out to be many diffrent types of levels...
