I think he collected a Shine Sprite and then got booted out of the level, but not into Peach's Castle? Definitely is weird.

The shine is in the middle of the huge rotating grass planet, and the starting is a bit far left from it. Probably it is a very strange kind of bug, although I'd like to hear a bit more about the before-and-after.

It happened to me once. I don't exactly remember how I did this, but I think I just grab the shine sprite and then switched the tab (I was using the browser version of SM63).

Switching tabs has a very weird affect on SM63, I can attest to that.

It might be flash related, Last Legacy likes to paint your level if you switch windows or tabs then click back.

Well, pausing the game before switching tabs should help, I do that a lot both when playing and designing

I was not playing on a browser… so switching tabs wasn't it.
I managed to fall after getting the shine and some land in a bottomless pit, and then this screen displayed.
When I clicked save, it acted like I was still in the level.

If you were playing the download version, you probably hit some kind of combination of buttons that messed with something, like Ctrl + Shift + Left or some similar combination.
