[GT] 4th Difficulty

I have taken down all the bosses on all difficulties by now, but still don't have the 4th difficulty. Do you have to buy it in the shop? Do you have to take down the final boss without upgrades?
How to unlock it?

I think you have to get all orbs, and get the second ending.
